To start the application process, please begin by downloading the waiting list application, HUD Publications, Receipt of HUD publications, 92006 Form, Disability Verification form, Release for Sex Offender and Criminal Background check. The Waiting list application must be filled out legibly and completely. Return of the Waiting list application, Receipt of HUD Publications, 92006 Form and Release for Sex Offender and Criminal Background check is required effective 7/1/2025 to be placed on the Waiting List. Waiting List Application Requirements:

  • The WAITING LIST application is mandatory for Fern Street Living Center, Kelley Street, Mark’s House, and College Park. This form begins the application process for Community Living Apartments (CLA), Community Living Center (CLC), and Fern Street Living Center (FS). However, for CLA, CLC, and FS, additional MSHDA/HUD-required paperwork must be completed to remain on the waiting list for these properties.

  • A CRIMINAL SEX OFFENDER RELEASE form must be submitted with all applications.

  • DISABILITY VERIFICATION is required prior to consideration for a unit offer. However, we may approve individuals for residency at College Park (CP), Mark's House (MH), and Kelley Street (KS), even if a doctor does not consider them disabled due to conditions like autism or other non-qualifying disabilities. The Disability Verification must be completed by a licensed professional qualified to make this determination and may be submitted later than the application.

  • The 9887 Form is required for FS, CLA, and CLC.

  • A STUDENT CERTIFICATION is needed for a unit offer at Fern Street, CLC and CLA, but it is not required at the time of the initial application.

  • For Fern Street Living Center applications, the MSHDA Checklist is required with the Waitlist Application.
